What if medical records were not confined to the point of care, but could be under a patient's care, with interfaces to institutional systems?
That idea is not new, and decentralized approaches like Diaspora 
or Federated Wiki go a long way in this direction.

I am particularly intrigued by the possibility that patients might pool their data (in responsibly managed environments of their choice), which could allow new kinds of research to be performed if large numbers are reached.
